Health Requirements for Summer School

State of Illinois law requires school districts to have certain health information on file for students entering Summer School including verification of immunizations as well as physical and dental examinations. Students who are not in compliance with the health form requirements will not be allowed to participate in Summer School at 911ÖÆÆ·³§ until requirements are met.
The school-based 911ÖÆÆ·³§ Health Center (H101) is available for exams and immunizations for eighth-grade students who will be attending 911ÖÆÆ·³§. Contact the Health Center at (847) 424-7265 to schedule an appointment.
Visit the Health Requirements and Forms web page to view or download school health forms. Please return the completed and signed "Illinois Certificate of Child Health Examination Form" to the Nurses' Office (N121) by May 1, 2025.
Forms can be dropped off to the Nurses' Office on school days during school hours, faxed, emailed, or mailed to:
