District 202 has purchased Student Accident Insurance Coverage on your child’s behalf.
This is not a primary coverage policy. This program provides supplemental coverage for your child for any injuries incurred while participating in any school-sponsored and/or supervised activity, including athletics.
NOTE: The accident policy benefits are limited and may not provide 100% coverage. Accident medical expense coverage under this policy is provided on an Excess Basis and benefits will only be paid under this plan after your own personal or group insurance has paid out its benefits. Completion of a claim form does not guarantee benefit payment. Each claim is reviewed according to the policy provisions.
ALL INFORMATION ON THE CLAIM FORM AND SUPPORTING DOCUMENTATION MUST BE PROVIDED IN ORDER FOR CLAIM TO BE PROCESSED. PROCESSING OF YOUR CLAIM WILL BE DELAYED IF COMPLETE INFORMATION IS NOT RECEIVED.
