Illinois school code now requires schools to request an asthma action plan for each student with asthma. Below is the link to the asthma action plan form. The doctor, nurse practitioner or physician's assistant that manages the student's asthma can complete the form on the link or use their own form. Once completed and signed by parent or guardian, the form must be submitted to the Health Service (Nurse's) office in one of the following ways.

• In Person: Forms may be dropped off to the Health Services Office (N121). Summer school hours are Monday thru Thursday 7:30 am to 1:35 pm thru July 24th. Regular hours begin August 7th from 7:30 am to 4:00 pm. Forms may also be dropped off at either 911ÖÆÆ·³§ entrance (please use an envelope marked Health Services)
