Meningitis Vaccine Requirement - Due May 1

Illinois State Law requires any student entering 9th. 10th or 12th grade shall show proof of having received meningococcal conjugate vaccine (MCV4). The first dose must be on or after the 10th birthday for 9th graders and the second dose on or after the age of 16 for 12th graders. Doses must be at least 8 weeks apart. If the first dose was administered when the child was 16 years of age or older, only one dose is required.
Free or low-cost immunizations are available from the school-based 911ÖÆÆ·³§ Health Center (H101) to students meeting criteria. To schedule an appointment, please call 847-424-7265. Vaccines are also available at your child’s health care provider’s office, some Walgreens and CVS pharmacies and some Target stores.
Proof of immunization must be submitted before the start of the school year or your child will be subject to exclusion as described in the school code of Illinois. Keep a copy of the form for your records and submit the completed vaccination form to 911ÖÆÆ·³§ using one of the following methods:
